This form is your festival summary report. Please note that Junior Students and Adult Students should each have their own festival summary reporting sheets. Please send 4 copies along with your check for the entry fees (total fee per student is $4.00).

This form is to report festival ratings. Please send me 2 copies of each form.

JR4-2

This form is to report students receiving cups. Please send me 2 copies.

JR4-1

This form is for Grand or Presidents Cup. Please send me 2 copies.

This form is for a 12 years of consecutive superior certificates. Please send me 2 copies.

Please be sure to fill out all forms correctly and send to: Sheri Neilsen, 726 Turner Road, Palmyra, NY 14522. All festivals must be completed by May 1st and all forms must be sent by May 5th.
