This form is your festival summary report. Please note that Junior Students and Adult Students should each have their own festival summary reporting sheets. Please send me 4 copies along with your check for the entry fees (total fee per student is $2.50). Please also send to me a self-addressed and stamped envelope addressed to the treasurer: Sylvia Smith, 81 Burdick Hill Road, Ithaca, NY 14850.

This form is to report festival ratings. Please send me 2 copies of each form.

This form is to report students receiving cups. Please send me 2 copies.

JR4-1

This form is for a 12-year cup winner. Please send me 2 copies.

This form is for a 12 years of consecutive superior certificates. Please send me 2 copies.

Be sure to send the order form with your check to me and then I will send it on to Zenia.

Please be sure to fill out all forms correctly and send to me: Troy Slocum, 34 Lewis Street, Geneva, NY 14456. All festivals must be completed by May 12th and your forms sent to me by May 15th. We can no longer have festivals beyond the May 15th date. If you need insurance please contact me as soon as possible.
